Safe Mission Hospice & Palliative Care, Inc. is looking for PER DIEM/PRN REGISTERED NURSE in the LA/OC area Beach Cities experienced in doing ADMISSION/s and Regular visit/s.

Hospice Registered Nurse is expected to report to clinical supervisor and other members of IDG including but not limited to: Administrator, DPCS, and/or Case manager whichever is applicable.
Experienced Hospice Registered Nurse plans, organizes, implements, and directs hospice care.
ESSENTIAL JOB FUNCTIONS & RESPONSIBILITIES:
- Provides direct patient care as defined in State Nurse Practice Act, including specialized duties related to end-of-life care.
- Develops a care plan that establishes goals, based on nursing diagnosis while making sure that the patient and the family are involved in the planning process.
- Provides health care instructions to the patient as appropriate per assessment and plan.
- Ensures that arrangements for equipment and other necessary items and services are available.
- Supervises ancillary personnel and delegates responsibilities when required.
- Participates in coordination of hospice services, appropriately reports identified needs or patient related issues l to the interdisciplinary group in a timely manner. This includes active participation in the biweekly interdisciplinary team meeting.
- Accurate documentation of hospice health related findings and patient services in a timely manner.
- Performs RN supervisory and re-certification visits in a timely manner.
- Demonstrates competency in nursing and assessment skills; able to accurately identify and communicate any changes of condition that warrant adjustments in the plan of care.
- Performs other duties as assigned by the Registered Nurse/Case Managers.
Required Qualifications:
- At least 1 year experience in Hospice within that last 3 years
- Current California RN license
- Understands and embraces philosophy of hospice and end-of-life. Also understands needs of the terminally ill.
- Good prioritization and communication skills.
- EMR: HospiceMD
- Valid CA Driver’s License and auto insurance
- Current CPR/BLS
- Current TB/Physical
- Pass Background Clearance
Preferred Qualifications:
- Bilingual (English/Spanish/Chinese/Vietnamese/Russian/Armenian)
- Baccalaureate degree in nursing
Pay structure is Pay Per Visit:00 - $85-250
