Job Opportunity: Full-Time Registered Nurse Case Manager – Hospice Care
Location: Southwest Connecticut
Our client, a well-established and rapidly growing hospice provider in Connecticut, is seeking a compassionate and dedicated Registered Nurse Case Manager to join their team. This is a full-time, permanent position with an immediate start.

What You Need to Succeed:
- Current RN License
- Valid Driver's License and Reliable Vehicle

Role Responsibilities:
- Patient Care: Conduct comprehensive physical and psychological assessments for patients referred for hospice care.
- Ongoing Care Management: Maintain regular nursing visits to ensure patients' needs are being met with the utmost compassion and professionalism.
- Territory Coverage: Serve patients across the Southwest CT region.
- Work Hours: Monday to Friday, 8:30 AM – 5:00 PM (with occasional on-call coverage).
