Job Summary
The Director of Case Management is pivotal in overseeing the case management department.

Currently accepting travelers that reside 50 miles outside of the facility location.

Position Details:

Position Name: Director of Case Management

Location: Exeter, New Hampshire

Length: 13 weeks-Accepting Travel Candidates only

Pay Rate: $3400 Weekly Gross pay (Non-taxable travel/housing and M&I stipend available for qualified/eligible travelers)

Unit: Case Management

Available shifts: Days

Shift timings: 8 hours, 8.00 am to 5.00 pm

Schedule: 40 hours/week guaranteed, 5 X 8-hr shifts, Monday-Friday, No weekends or On-call

Position Requirements

Director will oversee the operations and staff (approximately 24 staff) for Case Management, Social Services, on-site Spiritual Care services and Utilization Review.

  • 7 to 10 years of relevant experience: Required
  • Case Management managerial/supervisory experience - Required
  • Prior travel experience: Highly preferred
  • Covid vaccination proof - Required

Education, License and Certification Requirements:

  • BSN – Required
  • MSN – Highly preferred
  • RN License for State of New Hampshire or Compact - Required
  • BLS Certification from AHA - Required

Job Description:
Responsibilities include (but are not limited to):

  • Directing the work of the Case Management department in accordance with federal, state and local guidelines and regulations
  • Facilitates appropriate utilization of hospital and community resources to provide high quality, cost-efficient care, and achieve throughput, length of stay, and clinical quality goals
  • Builds and maintains strong working relationships with post acute partners to ensure successful transitions of care and positive patient outcomes
  • Reviews re-admissions and oversees opportunities to reduce preventable re-admissions; presents reports and statistics to the Utilization Review/Quality Oversight Committee
  • Responsible for the department’s budget, contracts with external vendors, and applicable policies/ standard operating procedures/ work instructions; plans and implements recruitment and retention strategies for areas of responsibility in collaboration with Human Resources.

Pay: $3,000.00 - $3,400.00 per week
