Position Name: Hospice Registered Nurse - Clinical Liaison

Pay Rate: Range $2000 to $2040 Weekly Gross pay (Non-taxable travel/housing and M&I stipend available for qualified/eligible travelers)
Mileage Reimbursement: $0.49/mile for distance covered between patient home appointments
Unit: Hospitals
Shift available: Days (8 hrs or 10 hrs)
Shift timings: 8 hour Days, 9.00 am to 5.30 pm or 10 hour days, 9.00 am - 7.30 pm
Schedule: 40 hours / week guaranteed. 5 X 8-hrs, 5 shifts per week, Tue to Sat (or) 4 X 10-hrs, 4 shifts per week, Wed-Sat
Position Requirements:
- Registered Nurse: 2 years - Required
- Recent/Current Hospice nursing experience in a home health setting: 1 year within the last 3 years – Required
- Hospice experience in a hospital or inpatient setting - Preferred in addition to the home health setting
- Hands on Home Care Home Base (HCHB) charting experience – Required
- Must have valid Driver’s License and Personal Auto Insurance
- Must be Covid Vaccinated or provide medical or religious exemption
Education, License and Certification Requirements:
- ASN or an equivalent – Required
- BSN or an equivalent – Highly preferred
- RN License for State of Ohio or Compact - Required
- BLS Certification from AHA or American Red Cross - Required
Job Description:
- Provide and coordinate care for assigned Hospice patients
- Develop and implement nursing care plans for patients as assigned.
- Empower patients and families to reach healthcare goals by educating them on disease management.
- Provide Home Health nursing care and services within the standards and scope of nursing practice as defined within the state Nurse Practice Act regulations, policies, procedures, and protocols, as well as facility and corporate policies and procedures.
- Ensure physicians’ orders are legible if taken verbally and transcribed appropriately per policy and procedure.
- Ensure patients’ status is assessed on an ongoing basis and pertinent information is gathered and documented.
- Notify physicians of significant changes in status.
- Document and complete charting
- Territories: Area of Coverage is Hospitals in Great Columbus and surrounding areas
