What are the responsibilities and job description for the MI-APPP Case Manager position at Saginaw Area Consortium?
- Participate with the referring worker/agency for all families to determine most appropriate services and on-going coordination of services for pregnant and/or parenting youth 21 years of age or younger.
- Develop positive relationships with families to encourage their active participation in program services.
- Motivate families to continue to participate in services.
- Complete required paperwork for all assigned families within the required time frames.
- Maintain face-to-face contact with families in accordance with project/contract requirements. Families will receive, at a minimum, two in-person home visits each month.
- Provide home visitation and home-based Adolescent Family Life Program-Positive Youth Development (AFLP-PYD) and Parents as Teachers (PAT) instruction.
- Facilitate groups geared towards young parents and their positive support persons.
- Coordinate outreach efforts to increase community awareness of the MI-APPP program.
- Maintain appropriate family records in accordance with project/contract requirements and internal policies, including: AFLP-PYD, PAT, Ages and Stages Questionnaires, program compliance forms, etc.
- Bachelor’s Degree in social work or other human service area is required.
- Experience working in a human service organization (paid and/or volunteer) is required.
- Previous experience working with families with prenatal and/or children birth to age five is preferred.
- Previous parent educator experience is preferred.
- Previous experience working with young parents, emphasis on young fathers preferred.
- Previous family support and self-sufficiency experience is preferred.
- Demonstrated ability to communicate effectively both orally and in writing.
- Must display initiative and enthusiasm in performance of essential job responsibilities.
- Must be non-judgmental, show compassion, empathy, and insightfulness in the performance of essential job responsibilities.
- Must be flexible in scheduling and able to work limited nights and weekends as needed and necessary.
- Parents as Teachers training is preferred.

GENERAL SUMMARY OF POSITION: The MI-APPP Case Manager shall follow established parent educator policies in working with identified families to strengthen their parenting skills and child development knowledge through home visitation, educational groups, and other service delivery methods. The MI-APPP Case Manager will work with identified families toward stability and self-sufficiency through connecting families with education, employment, health care, and other resources in the community. Additionally, the MI-APPP Case Manager will monitoring family progress by assisting in goal development, managing barriers and celebrating family successes.
