Summary
Responsible for identifying eligible participants and then scheduling and conducting home visits and plumbing lead assessments. Provide clients with education related to safe home plumbing maintenance. Coordinate referrals for faucet replacements as well as other lead prevention services if applicable.
Essential duties and responsibilities
· Provide case management services by assessing the multiple or complex needs and/or inadequate assistance from family members or other informal supports.
· Facilitate wrap-around services provided by the agency with regular follow-up.
· Schedule and conduct home visits and perform home and plumbing lead assessment along with related education about safe plumbing maintenance.
· Build and maintain collaborative relationships with area service providers.
· Make external referrals and follow up to ensure the client need was met.
· Assist and advocate for support services from area service providers.
· Enter client case information into CRM databases (e.g., MDHHS systems, EmpowOR)
· Conducts client home visits to provide ongoing support services
· Prepare and submit reports of activities weekly via email or as requested by MDHHS.
· Maintains accurate files and records of daily activities and client intervention.

· Must be able to work with technology to enter client information and take pictures of faucet repair needs in the home.
· Knowledge of community agencies/partners who provide supports for the home plumbing program
Education and/or Experience
· Bachelor’s degree or equivalent in Human Service or related field. (required)
· Minimum 1 year experience in case management or non-profit family support service (required)
Certificates, Licenses, Registrations
- Must have reliable insured transportation and a valid Michigan Driver License
Job Type: Part-time
Pay: $17.50 - $25.00 per hour
Expected hours: 28 per week
