POSITION TITLE: Adult Case Manager

RESPONSIBLE TO: Adult Case Manager Supervisor

DEPARTMENT/SITE: SCCMHA / Saginaw

STATUS: Fee for Service; Unit requirement.

All the criteria-based duties and standards within this document will be performed according to established policies, procedures and guidelines within the department/site and SPSI.

JOB SUMMARY

Case managers are healthcare professionals who help people with a large variety of issues. They assist both individuals and families in coping with complex physical, mental health and substance use conditions. Case managers are there to find the most effective treatment with an integrated approach.

Case managers help people who are in difficult situations figure out what kind of help they need, help them find the services they need, create plans for treatment or recovery, work with other health and human service providers, and track client's progress with treatment plans. Case managers work in many aspects of people's lives, and may work with children and families, senior citizens, those with disabilities, recovering addicts, the mentally ill, immigrants, ex-offenders or the homeless. The relationship with clients is an advocate. The development of such a relationship is a key step in successful case management.

ESSENTIAL JOB DUTIES

1. Reach out to clients assigned by supervisor, asses their most urgent needs, appraise the situation, and listen to their concerns.

2. Perform an in-depth mental and physical health analysis of the client/family.

3. Develop a detailed plan of action to meet the client’s needs, set goals, and find the necessary resources to meet those goals.

4. Offer comprehensive psychosocial services to patients in either individual or group settings.

5. Consult with external agencies in order to provide clients with support services and resources.

6. Keep comprehensive records of clients’ progress throughout the process, including every call, referral, and home visit.

7. Maintain the confidentiality, respect the privacy, and preserve the routine and independence of clients as much as possible.

8. Help clients in meeting basic needs for food, clothing, shelter, personal safety, and general medical and dental care as well as assist with applications for income, medical, housing or other benefits which they may need and are entitled.

9. Work in accordance with clinical therapists’ medical staff and peer-parent support specialists.

10. Stay in touch with clients to ensure the services were beneficial, and that their needs are met.

11. Engages and develops trusting relationships to enhance successful mental health/substance abuse outcomes using educational, behavioral, and motivational interventions.

12. Accurately chart observations on assigned clients using correct charting format and terminology.

13. Assists in coordinating individual treatment planning including aftercare and recovery support services for each client actively involved in mental health/SUD treatment.

14. Assists each client receiving mental health/SUD treatment in becoming involved with self-help support groups and developing and maintaining social support networks.

15. Reliable in carrying out assignments conscientiously.

16. Continually completes notes, assignments and incident reports on time per policy.

17. Maintain your credentials and required trainings to satisfy your licensure and/or insurance.

MARGINAL JOB DUTIES

1. Attend all mandatory facility in-services and staff development activities as scheduled.

2. Give appropriate feedback regarding the condition of clients to your supervisor.

3. Coordinate with different professionals and agencies as a team to ensure the consumer’s plans are up to date on meeting their needs as needed.

4. Conduct crisis intervention as needed and seek appropriate guidance when necessary.

JOB SPECIFICATIONS

EDUCATION: Must have a bachelor in social work equivalent in related field areas such as, psychology, professional counselor, social work, and/or CAADC/CADC (if applicable).

LICENSURE: Valid driver’s license, bachelor level social work license (if applicable).

EXPERIENCE: A minimum of one to three years’ experience in case management in a mental health or substance use disorder (SUD) setting required.

1. Self-motivated and the ability to handle unforeseen problems. Does not wait to be told what to do, can work independently.

2. Exceptional organizational and leadership skills.

3. Aptitude in resolving issues and conflicts.

INTERPERSONAL SKILLS

1. Must possess effective communication skills and act as a representative of SPSI and adhering to its mission, vision, and values. This includes external representation of local agencies.

2. Speak to clients in a respectful and calming manner, even if clients are agitated. Identify and resolve client conflicts/dissatisfaction issues as needed.

3. Engages and develops trusting relationships with clients to enhance successful mental health/substance use outcomes.

4. Continually strives for effective relationships with staff, clients and management.

Expect self and co-workers to maintain the highest standards of professional ethics.

PHYSICAL EFFORTS: Medium; must be able to lift up to 50 lbs.

HOURS OF WORK: Full-time, flexible and varied. Schedule may vary Monday through Friday 8 a.m. to 7 p.m.

TRAVEL: Reliable transportation is required. Local travel throughout service area is required with possible occasional travel between SPSI locations. Some out of service area travel may also be required. Reimbursement in accordance with SPSI policy.
