Job Summary
Sai Adolescent Residential Treatment Center is a treatment program for adolescents ages 12-17 years of age with Mental Health and/or Substance Use challenges. The youth live at the program for up to 90 days and receive Mental Health Therapy, Substance Use Counseling, Art Therapy, Yoga Therapy, Music Therapy, Online Education and many other services.
We are in search of a dynamic person to fill the role of the Intake Specialist and Case Manager. We are searching for someone who has experience in the Behavioral Health field or is interested in pursuing a career in the field. We prefer to hire someone who has experience working with challenged adolescents and shows compassion and caring treatment to our residents.
The Intake Specialist plays a crucial role in the initial assessment and support of clients seeking services. This position involves gathering relevant information, evaluating client needs, and facilitating access to appropriate resources. The Intake Specialist will work closely with individuals from diverse backgrounds, ensuring a compassionate and effective approach to service delivery.
Duties
- Conduct thorough assessments of clients' needs and circumstances to determine eligibility for services.
- Collect and document client information accurately and confidentially.
- Collaborate with various departments to ensure seamless service delivery and follow-up.
- Provide crisis intervention support as needed, ensuring clients receive immediate assistance.
- Facilitate referrals to appropriate programs, including behavioral health services, workforce development, and addiction counseling.
- Maintain up-to-date knowledge of community resources
- Participate in ongoing training and professional development opportunities to enhance skills.
Case Manager Responsibilities:
- *Enhancing community integration through increased opportunities for access and involvement; and creating opportunities to enhance community living skills that promote community adjustment, including the use of local community resources available to the general public to the greatest extent possible.
- Implementation of individualized services plan.
- *Assessing needs and planning services to include developing an individualized case management service plan.
- *Linking the individual to those community supports that are likely to promote the personal habilitative/rehabilitative and life goals of the individual as identified in the individualized service plan.
- *Directly assisting the individual to locate or obtain needed services, resources and appropriate public benefits.
- *Assuring the coordination of services among providers and with other human service agencies and systems, such as local health and social services departments.
- *Monitoring service delivery through contacts with youth and family and service providers through periodic site and home
- *Maintains complete and current required reports, service reporting and daily documentation in compliance with agency, state and federal standards.
- *Provide psycho-education and counseling to guide the individual and develop a supportive relationship that promotes the implementation of the individualized service plan.
- *Advocating for individuals in response to their changing needs.
- *Developing a crisis plan for an individual that includes the individual’s preferences regarding treatment in an emergency situation.
- *Assisting with planning in developmental transition periods
- *Knowing and monitoring the individual’s health status, medical conditions, and medication benefits and side effects, and assisting the individual in accessing primary care.
- *Other duties as assigned by the supervisor in keeping with the general requirements of the position.
- Valid Driver’s License and own transportation which is safe and can be used for work-related travel.
- Able to perform CPR and first Aid when a situation requires these activities
Skills
- Strong background in social work or behavioral health is preferred.
- Proficient in crisis intervention techniques and mentoring practices.
- Knowledge of workforce development strategies and community resources.
- Ability to provide addiction counseling or support related to childcare issues is beneficial.
- Excellent communication skills, both verbal and written, with a focus on empathy and active listening.
- Strong organizational skills with the ability to manage multiple tasks effectively.
Job Type: Part-time
Pay: $20.00 - $21.00 per hour
Expected hours: 15 – 20 per week
