adult based critical care and acute care RT.
Hamilton ventilator training would be ideal.
Job Requirements
American Heart Association certification is required for all direct patient care personnel.
Must include an image of the front of the card as well as an image of the back of the card.
Must be dated within 30 days of start.
Attach scanned online verification page with a machine-generated date indication when the verification was performed within the past 30 days.
9 Panel completed within 30 days of start date, preferred within 14 days of start.
All engaged contractors must have page 27 signed form on file in Trinity VMS.
Standard i9 completed by the agency.
Trinity Health Confidentiality of Patient and Proprietary Information and Information System Security Agreement must be signed.
Proof of vaccine, declination or exemption (with agency letterhead).
Must have 1st vaccination in series prior to start; 2nd shot in series 30 days later; 3rd shot in series 6 months later.
