**Certification Requirements**
We are seeking a Respiratory Therapist for a 13-week travel position at Saint Alphonsus Regional Medical Center.
To be eligible for this role, you must have the following certifications:
- American Heart Association (BLS) certification is required for all direct patient care personnel.
- Licenses and certificates must be verified within 30 days of start date.
- Fitness test is required.
- You must complete Trinity 9 Panel Drug Screen within 30 days of start date.
- Trinity Code of Conduct Form must be signed.
- Trinity I-9 form must be completed.
- Trinity Confidentiality of Patient and Proprietary Information Agreement must be signed.
- Trinity Integrity and Audit Services Form must be completed.
- Trinity Specialty Specific Competency Testing must be completed within a year.
- Color Vision testing must be completed.
- Influenza vaccine proof is required.
- 2 Step PPD or IGRA (Quantiferon/Tspot) is required.
- COVID-19 vaccine proof is required.
- MMR vaccine proof is required.
- Hepatitis B vaccination proof is required.
- Varicella vaccination proof is required.
- Tdap vaccination proof is required.
- A physical/health statement is required.
- Your resume and skills checklist must be provided.
**Additional Requirements**
Please attach a scanned copy of your American Heart Association certification to your record in TrinityVMS. The certification must include an image of the front and back of the card.
