JOB DESCRIPTION
SUBJECT: PASTORAL CARE ASSOCIATE REWRITTEN: 2022
ESSENTIAL FUNCTIONS:
1. Makes initial visit with residents as soon as possible after admission and completes admission assessment.
2. Notifies resident’s church of their admission to Saint Anne Communities (SAC), if resident wishes, to provide continuity for their spiritual needs.
3. Visits all residents on all floors and maintains a current updated file of resident visits and significant events and other pertinent resident information.
4. Provides spiritual programming for the residents of SAC. This includes Bible studies, prayer groups and so forth.
5. Responsible for scheduling Chapel services, Roman Catholic and Protestant, as well as posting this schedule. This also includes arranging special services for holy days and special seasons.
Communication to appropriate staff and residents must also be maintained.
6. Offers Communion to residents unable to attend Mass.
7. Offers spiritual consolation to residents and their families at time of death or serious illness through prayer and supportive ministry.
8. Assists in the planning and conducting of Memorial Services.
9. Coordinates funeral services for residents with the resident’s family, the staff at SAC, and the funeral director when the funeral is a SAC
10. Responsible for coordinating volunteer services to ensure resident’s spiritual needs are met daily.
11. Responsible for providing a morning and noontime prayer using the intercom system for the Facility.
12. Maintains chapel appearance and supplies including candles, hosts and wine and arranges for chaplain to bless.
13. Gives Pastoral Care orientation to new employees when possible.
14. Available for pastoral care to staff as needed.
15. Responsible for publishing the bulletin and The Messenger weekly.
OCCASIONAL FUNCTIONS:
1. Visits residents who are in the hospital.
2. Leads Protestant Bible study and/or worship service as needed.
3. Posts, on designated bulletin board, the names of residents who have died and keeps board up-to-date and informs staff using e-mail of resident’s funeral arrangements.
4. Responsible for check request made on the first of each month for organist, Catholic chaplain and
Protestant chaplain as well as stipends for Masses prayed for deceased Catholic residents.
5. Collects and counts Sunday collection and vigil money.
6. Helps with Activities as needed.
7. Represents SAC at calling/viewings of residents having services in the Fort Wayne area.
JOB QUALIFICATIONS:
A. Must be an active member of the Roman Catholic Faith and have Parish ministry experience
B. Must have an understanding and interest in meeting the spiritual needs of SAC residents.
C. Must be knowledgeable about religious traditions and preparation needed for these.
D. Ability to communicate with volunteers effectively and coordinate their services.
E. Ability to coordinate scheduled religious events.
