What are the responsibilities and job description for the Care Manager position at Saint Dominic's Family Services?
The Care Manager is responsible for the Care Coordination of Targeted Case Management Cases. This includes assessment of new referrals, outreach to gain referrals and coordinating medical and mental health services with assigned Health Home.
Must be willing to travel to provide case management for families in Rockland & Orange Counties.
Education & Experience
Bachelor Degree in Human Services. Two (2) years experience providing direct services to Severely Emotionally Disturbed (SED) children and adolescents, or providing linkage to services for children with one mental health diagnoses.
Job Type: Part-time
Pay: $25.27 - $27.47 per hour
Expected hours: 19 per week
