Job Summary: The Referral Coordinator describes hospice philosophy and services to patient and families and physicians to ensure goals for care are consistent with Saint Francis philosophy of care.
Minimum Education: Has completed the prescribed curricula of a practical nursing program or has completed equivalent coursework as approved and verified by a state board of nursing and holds or is entitled to hold a diploma or certificate therefrom.
Licensure, Registration and/or Certification: Valid multi-state or State of Oklahoma Licensed Practical Nurse License.
Work Experience: At least 1 year as a nurse in a clinical setting with previous experience in discharge planning and/or Medicare experience preferred. Knowledge of community agencies which provide assistance and support to the community.
Knowledge, Skills and Abilities: Demonstrates teaching abilities. Good interpersonal and communication skills.
Essential Functions and Responsibilities: Initiates patient referral/admitting process. Ensures patient/family and physicians goals for cares are palliative in nature. Facilitates communication between hospice team members. Facilitates and coordinates insurance verification information and communication from insurance case managers to other health team members. Supports agency/hospital goals through QA and educational activities.
Decision Making: Independent judgment in making decisions involving non-routine problems under general supervision.
Working Relationships: Works directly with patients and/or customers. Works with internal/external customers. Works with other healthcare professionals and staff. Works frequently with individuals at director level or above.
