JOB SUMMARY
The Health Home Care Coordinator will be responsible for addressing the health and social service needs of patients and their families by helping them to navigate and access community services and resources, and to adopt healthy behaviors. The Health Home Care Coordinator supports providers and Nurse Care Managers though an integrated approach to care coordination and community outreach. Specifically, the Health Home Care Coordinator will promote, maintain, and improve the health of patients and their families; provide social support; and advocate for health needs.
JOB DETAILS AND REQUIREMENTS
Type: Full Time (80 hours per 2 week pay period, with benefits)
Typical Hours: Day Shift, 8 hours/shift, Monday - Friday
Education:
High School Diploma Required
Certification & Licensures:
BLS Certification
Experience:
Previous experience with Electronic Medical Records-preferred.
Proficiency in basic computer use-required.
