Certification Details BLS State Licensure PALS ACLS TNCC Additional Details Proof of vaccine Sept 2024 booster, declination or exemption(with agency letterhead). Declination not accepted for CA or IL. Attached scanned proof of 2 vaccinations with dates or primary source IgG titer with lab print-out to candidate's record in Trinity VMS. Must have 1st vaccination in series prior to start; 2nd shot in series 30 days later; 3rd shot in series 6 months later. Hep B titers will NOT be accepted. Proof of vaccination must be uploaded into Trinity VMS. Proof of vaccine, declination or exemption(with agency letterhead). Declination not accepted for CA or IL. Flu dates : 10 / 1 / 2024-4 / 30 / 2025. Attached scanned proof of 1 vaccination with date or primary source IgG titer with lab print-out to candidate's record in Trinity VMS. Upload a current, non-expired TDAP vaccine. Declinations are accepted if the assignment does not require Pedatric interaction. Agency form acceptable for declination. Provide documentation from an exam within the last year that candidate is fit to work. Attach scanned copy of certification to candidate's record in Trinity VMS. 9 Panel completed within 30 days of start date, preferred within 14 days of start. Must be completed and resulted before start with MRO signature. Must include the following : Cocaine, Amphetamines, Opiates, 6- Monoacetylmorphine, Propoxyphene, PCP, Barbiturates, Benzodiazepines, Methaqualone, and Methadone. (NO THC).
