Valid for 2 years. Available for free at Respirator fit test. Locations that allow onsite testing : NJ - Saint Michael's Medical Center, NJ-Saint Mary's Hospital (Mask Accepted : 3M 9205 ), CA-West Anaheim Medical Center, CA-Garden Grove Hospital Medical Center.
Flu Vaccine or declination required October 1-May 31. Declinations not accepted at Shasta Regional Medical Center.
