Director of Social Services (LCSW / LICSW)
Responsibilities of the Director of Social Services
- Directs and monitors the social work services in the center.
- Promotes the psychosocial wellness of the residents.
- Actively participating in resident assessments and discharge planning to allow the resident to optimize healthcare benefits.
- Works closely with the Interdisciplinary Team with regards to the social wellbeing or the resident.
- Coordinates and arranges for resident's care planning meetings ensuring that the resident and responsible persons are involved as able.
- Works closely with community resources such as Home Health, Hospice, local housing authorities, shelters, and day treatment providers to ensure resources are available as needed per discharge plan.
- Discusses with resident and family members / responsible party regarding Advanced Directives.
- Thoroughly and accurately complete assessments
- Supervises the second social worker in the department.
