About the Role
The Continuing Care Coordinator plays a vital role in supporting the mental health and well-being of clergy and religious individuals at Saint John Vianney Center. This position requires a compassionate and organized individual who can work effectively with a diverse range of stakeholders.
Key Responsibilities
- Provide ongoing case management services to residents, including discharge planning and aftercare coordination.
- Build and maintain relationships with referral sources to ensure smooth transitions for residents.
- Develop and implement care plans that meet the unique needs of each resident.
- Maintain accurate records and reports to ensure compliance with regulations.
Requirements
- Bachelor's degree in Social Work, Case Management, or a related field.
- At least two years of experience in residential or hospital settings working with adult psychiatric residents/clients.
- Valid driver's license and ability to obtain driving approval from SJVC.
- Ability to travel locally and overnight when necessary.
- Knowledge of and experience working with clergy and religious preferred.
- Excellent communication and interpersonal skills.
