Job Summary
We are seeking a qualified Continuing Care Coordinator (CCC) to assist residents with ongoing case management, discharge planning, and aftercare services. The ideal candidate will have a well-rounded perspective on comprehensive patient care and offer excellent customer service.
Responsibilities
- Assist residents with ongoing case management while in treatment
- Conduct discharge planning and coordinate aftercare services
- Build and foster long-term relationships with referral sources
- Conduct on-site meetings for each resident on their caseload
Requirements
- Degree in Social Work, Case Management, Health and Human Services, or related field; preferably two years' experience
- Demonstrated knowledge and two years' experience in residential or hospital settings working with adult psychiatric residents/clients
- Valid Driver's License and ability to obtain Driving Approval from SJVC
- Ability to travel locally and overnight when necessary, up to 40%
- Knowledge of and experience working with Clergy and Religious preferred
- Ability to serve as a trusted resource for behavioral health information following HIPAA guidelines
- Excellent customer service and interpersonal skills, with the ability to work as part of a team
- Willingness to support and respect all aspects of the Catholic Church
- Strong communication skills required, with written, verbal, and non-verbal abilities
- Spanish speaking a plus
- Strong organizational and time-management skills necessary to maintain a demanding workload
- Familiarity with Microsoft Office Suite and electronic health records (EHR) preferred
