Job Summary
We are seeking a highly skilled Continuing Care Coordinator to join our team at Saint John Vianney Center. The successful candidate will be responsible for assisting residents with ongoing case management, discharge planning, and aftercare services.
Key Responsibilities
- Conduct on-site meetings with residents to discuss their caseload and provide support.
- Work closely with referral sources to build long-term working relationships.
- Coordinate treatment and aftercare services to ensure seamless transitions for residents.
- Maintain accurate records and reports to ensure compliance with regulations.
Requirements
- Bachelor's degree in Social Work, Case Management, or a related field.
- At least two years of experience in residential or hospital settings working with adult psychiatric residents/clients.
- Valid driver's license and ability to obtain driving approval from SJVC.
- Ability to travel locally and overnight when necessary.
- Knowledge of and experience working with clergy and religious preferred.
- Excellent communication and interpersonal skills.
