Continuing Care Coordinator Responsibilities
The Continuing Care Coordinator plays a vital role in ensuring seamless transitions and successful outcomes for our residents. Key responsibilities include:
- Collaborating with multidisciplinary teams to develop individualized care plans
- Coordinating with referral sources to establish and maintain strong relationships
- Providing education and support to residents and their families
- Facilitating on-site meetings and engaging residents in goal-setting and planning
Requirements and Qualifications
- Degree in Social Work, Case Management, Health and Human Services, or related field
- Minimum two years' experience in residential or hospital settings working with adult psychiatric residents/clients
- Valid Driver's License and ability to obtain Driving Approval from SJVC
- Ability to travel locally and overnight when necessary, up to 40%
- Knowledge of and experience working with Clergy and Religious preferred
- Strong communication and interpersonal skills, with a focus on empathy and understanding
- Ability to work effectively in a fast-paced environment and prioritize tasks efficiently
