Saint John 23rd Home is seeking a compassionate and dedicated Social Worker to join our team. The ideal candidate will be committed to providing high-quality care and support to our residents, helping them navigate emotional, social, and psychological challenges while maintaining dignity and respect.
Key Responsibilities:
- Provide individualized psychosocial assessments and interventions for residents and families.
- Develop and implement care plans that address emotional, social, and environmental factors impacting residents.
- Offer counseling and support services to residents and families, helping them adjust to the challenges of living in a skilled nursing facility.
- Advocate for resident needs and assist with the coordination of resources within the facility.
- Work closely with nursing, therapy, and medical staff to ensure holistic care for each resident.
- Facilitate support groups and provide resources for residents and families dealing with grief, loss, or other emotional challenges.
- Assist residents in applying for government benefits (e.g., Medicaid, Medicare) and other resources.
- Maintain accurate and timely documentation in compliance with facility policies and regulations.
- Ensure compliance with all state and federal regulations regarding social work services in long-term care settings.
Qualifications:
- Bachelor's or Master's degree in Social Work (BSW or MSW) from an accredited program.
- Current licensure as a Social Worker in the state of Pennsylvania
- Previous experience in a skilled nursing facility or long-term care setting is preferred.
- Strong interpersonal and communication skills.
- Ability to work effectively as part of an interdisciplinary team.
- Knowledge of state and federal regulations related to skilled nursing and long-term care services.
