Saint Luke's is hiring for a Nurse Care Coordinator for their Population Health team at the East Location. This Registered Nurse will provide care to a diverse patient population.
Hours: Monday - Friday, 8am - 5pm
The Work:
- We are looking for an experienced nurse who is passionate about advocating for their patients and providing patient education to help them understand how they can impact their own health and be part of the healing process.
- This is an autonomous role that is supported by a strong comprehensive team.
- We are seeking a RN who can see the big picture and will treat each patient uniquely to ensure individualized care is provided. No two patients are the same.
Responsibilities:
- The Care Coordinator Patient Centered Medical Home is responsible for the application of extensive knowledge in clinical performance improvement activities.
- This individual will be responsible for driving engagement strategies for improving physician and staff patient care practices based on the Patient Centered Medical Home model clinical guidelines best practices, internal and external benchmarks, and claims based data.
- The PCMH Coordinator supports and ensures the overall success of the PCMH model by collaboratively working with patients, physicians, practice teams and health plans to select, guide and direct patient care, while integrating key features of the medical home.
