Responsibilities

St. Mary's Medical Center Physician Care is now hiring a full time day shift Licensed Practical Nurse (LPN) Care Manager to join our team! The Licensed Practical Nurse (LPN) - Care Manager duties include, but are not limited to :

act as a liaison between patients, health care providers, and health insurers to ensure patients receive excellent health care

provide direct and indirect patient care services requiring technical and manual skills, within the scope of practice, that meet the psychosocial, physical and general aspects of care

maintain patient medical records and keep track of health outcomes

communicate the needs of the patient and family

consider the patient's unique care needs and advocate for personalized treatment options

develop and manage long-term care plans for patients with chronic or serious conditions

educate patients and their families on relevant health-related matters, and offering guidance for navigating complex medical decisions

performs under the direction and supervision of a Registered Nurse

have exceptional communication skills, organizational skills, and analytical and problem-solving skills

Qualifications

Education and Work Experience

current and valid state license as a Licensed Practical Nurse (LPN / LVN)

current BLS (AHA) certificate upon hire and maintain current

minimum one year experience in an applicable setting, preferred

IV certification upon hire
