Description
The role of the Discharge Planner at Saint Simeon’s is to support the residents/patients and their respective families, by developing open dialogue at the beginning of their admission about what their goals are for discharge and assisting them to meet those goals or modified the discharge along the way if their goal is no able to be met. The Discharge Planner will conduct themselves in accordance with current federal, state, and local guidelines and regulations, as well as Saint Simeon’s established policies and procedures, and as directed by the Administrator and or President/CEO.
- Assist in the planning, developing, organizing, implementing, and evaluating of the Social Service Program of this facility.
- Keep abreast of current federal and state regulations, as well as professional standards and make recommendations on changes in policies and procedures to the Administrator and/or President/CEO.
- Assist in developing and implementing policies and procedures for the identification of the discharge planning needs of the resident.
- Participate in community planning related to the interests of the facility and the services and needs of the resident and family.
- Participate in development and implementation of discharge care plans and resident assessments, if directed.
- Interview resident/family as necessary and in a private setting.
- Involve the resident/family in planning objectives and goals for the resident.
- Perform administrative requirements, such as completing necessary forms, reports, etc. and submitting such to the Administrator as required.
- Issue NOMNC’s in a timely manner according to state and federal regulations.
- Provide clinical documentation upon admission, periodically when there are changes, and upon discharge.
- Maintain a productive working relationship with the medical professions and other health related facilities and organizations.
- Coordinate services with other departments as necessary.
- Coordinate services with 3rd party providers and DME companies to coordinate necessary equipment for discharge.
- Keep abreast of economic conditions/situations and recommend to the Administrator adjustments in the discharge program that assure the continued ability to meet resident needs.
- Participate in surveys (inspections) made by authorized government agencies.
- Meet with administration, medical and nursing staff, as well as other related departments in planning resident discharge.
- Other duties as deemed necessary and appropriate, or as may be directed by the Administrator and/or President/CEO.
Qualifications

- Case Management certification preferred
