We focus is on managing a set of patients with complex and expensive health issues such as polypharmacy, infectious disease treatment, and chronic diseases like heart failure. We are looking for a dedicated Clinical Program Manager to join our team and lead our efforts in these critical areas
Job Summary: The Clinical Program Manager will be responsible for designing, implementing, and overseeing clinical programs aimed at improving patient outcomes and avoiding unnecessary admissions. This role requires a strategic thinker with a strong clinical background and experience in managing long-term care patients.
Key Responsibilities:
- Design and develop clinical programs to address issues such as polypharmacy, high-risk patients, and chronic disease management.
- Oversee the implementation and evaluation of these programs to ensure they meet clinical and operational goals.
- Collaborate with healthcare providers, nursing homes, and other stakeholders to ensure the programs are patient-centered and effective.
- Monitor and analyze program performance data to identify areas for improvement and implement necessary changes.
- Provide leadership and guidance to clinical staff involved in the programs.
- Stay up-to-date with the latest clinical guidelines and best practices in long-term care and chronic disease management.
Qualifications:
- Bachelor's degree in Nursing, Healthcare Administration, or a related field (Master's degree preferred).
- Minimum of 5 years of experience in a clinical setting, with at least 2 years in a management role.
- Experience with long-term care patients is a big plus.
