Your Role With Us:
As a Care Manager at Salem Health, your role involves providing comprehensive care coordination, utilization management, and discharge planning for assigned patients. You will begin by conducting an initial screen of all patients within 24 hours of admission to assess their needs, identifying high-risk categories and ensuring timely interventions. Throughout the patient's care journey, you will collaborate with patients, families, and multidisciplinary teams to create tailored care plans, monitor progress, and address any barriers to discharge or transfer. You will also serve as the primary point of contact for Swing Bed referrals, overseeing the coordination of care from admission to discharge, including managing insurance authorizations and creating individualized care plans. Your responsibilities include enhancing patient outcomes by reducing readmissions and emergency visits, facilitating the transition of care across various settings, and educating staff on care management principles. Additionally, you will document patient information in accordance with department policies and contribute to improving the care delivery process by participating in peer consultation and decision-making activities.

Background/Expertise:
- Associate Degree of Nursing (ADN)- ADN with 5 years of experience working of acute care RN experience OR ADN with an agreement to complete BSN or MSN within two (2) years of hire REQUIRED
- BSN or BAN or MSN of Non nursing Bachelor's degree with MSN may be accepted in lieu of ADN
- Active Basic Life Support Credentials REQUIRED
- Registered Nurse- Current unencumbered licensure to practice nursing as a Registered Nurse in the state of Oregon REQUIRED
- Certified Case Manager credentials REQUIRED
- Minimum of one (1) year of acute care RN Staff nurse experience REQUIRED
- Minimum three (3) years hospital experience as a Registered Nurse REQUIRED
- Two (2) years of community nursing PREFERRED
- Prior Case Management experience PREFERRED
