Your Role With Us:As a Care Manager at Salem Health, your role involves providing comprehensive care coordination, utilization management, and discharge planning for assigned patients. You will begin by conducting an initial screen of all patients within 24 hours of admission to assess their needs, identifying high-risk categories and ensuring timely interventions. Throughout the patient's care journey, you will collaborate with patients, families, and multidisciplinary teams to create tailored care plans, monitor progress, and address any barriers to discharge or transfer. You will also serve as the primary point of contact for Swing Bed referrals, overseeing the coordination of care from admission to discharge, including managing insurance authorizations and creating individualized care plans. Your responsibilities include enhancing patient outcomes by reducing readmissions and emergency visits, facilitating the transition of care across various settings, and educating staff on care management principles.
