Position: Full Time Wound, Ostomy, and Continence Nurse/Utilization Review/RN Case Manager

PURPOSE
The Wound, Ostomy, and Continence Nurse/UR/RN Case Manager serves as a role model for professional nursing and assists with ensuring that the needs of the patient with a stoma, impaired skin integrity, and urinary problems, are being appropriately met. This may include educating physicians, nursing staff, or collaborating with other interdisciplinary team members. This role must have good communication and problem-solving skills. On days when the role is more UR/Case Management focused the nurse will
conduct medical certification review for medical necessity for acute care facility and services. They will use nationally recognized, evidence-based guidelines approved by medical staff to recommend a level of care to the physician and serve as a resource to the medical staff on issues related to admission qualifications, resource utilization, national and local coverage determinations, and documentation improvement activities. The RN Case Manager will assist with discharge planning and address any needs a patient may have on discharge.

QUALIFICATIONS
- Graduate of an approved professional school of nursing and currently licensed as a Registered Nurse in the State of Ohio.
- Successful completion of an accredited course in Wound, Ostomy, and Continence. Board certified in Wound, Ostomy, and Continence through the WOCN-CB, or extensive experience in leadership of a wound care clinic.
- Maintains certification in BLS and ACLS.
- Maintains required continuing education for certification and licensure.
- At least three-five years of nursing experience, preferably including critical care.
- Working knowledge of general computer skills.
- Expert knowledge of MCG or InterQual Level of Care Criteria and knowledge of local and national coverage determinations
- Hospital Case Management Experience preferred
- Eligibility to remain in position requires a 90% proficiency rating based on chart audits.
