As a Medical Social Worker, you will play a vital role in supporting patients and their families through health-related challenges by:
- Conducting comprehensive assessments of patients’ social, emotional, environmental, financial, and support needs.
- Developing tailored plans of care to address identified needs and ensure the best outcomes.
- Providing critical intervention for patients facing complex challenges, including chronic illness, terminal illness, mental illness, homelessness, or multiple social and financial difficulties.
- Collaborating with healthcare teams and community resources to provide compassionate and effective support.
- Advocating for patients and empowering them to navigate their healthcare journey.

What You’ll Do:
As a Medical Social Worker working PRN - on call as needed at HRRMC, you will:
- Conduct comprehensive psychosocial assessments and support patients and families in understanding illness and treatment options.
- Help patients adjust to hospitalization and treatment, providing emotional and social support.
- Educate patients and families on community resources, entitlements, and advance directives.
- Offer crisis intervention, grief counseling, and referrals for mental health care.
- Educate staff on psychosocial issues and guide decision-making in patient care.
- Coordinate discharge planning, continuity of care, and resources for medications and equipment.
- Advocate for patients in inpatient, outpatient, and community settings.
- Participate in Swing Bed discharge planning and ensure accurate documentation.
Your role is key to helping patients and families navigate their healthcare journey with compassion and support.
Qualifications:
To excel in this role, you’ll need the following skills, experience, and attributes:
- High attention to detail, ensuring accuracy in all tasks.
- Proficiency in state-of-the-art office technology, including the Microsoft Office Suite and other relevant tools.
- Strong ability to handle confidential and sensitive information with professionalism, tact, discretion, and integrity.
- Ability to work both independently and collaboratively, adapt to changing priorities, and demonstrate resourcefulness and strong problem-solving skills.
Education and Experience:
- A Bachelor's or Master's degree in Social Work.
- At least 1 year of experience in social work (LCSW preferred).
