POSITION SUMMARY
Position Summary:
Responsible for reviewing, clarifying, and documenting home medication lists of all patients admitted to the hospital. May also be required, under the supervision and direction of a pharmacist, to fill medication orders and perform related duties; and transport medications. The Medication Reconciliation Tech performs duties functioning in accordance with national and state standards, laws, and practices. Assists pharmacists as directed.
POSITION QUALIFICATIONS
Minimum Education
High school diploma or GED (or eight years equivalent experience)
Minimum Experience
Two years related experience preferred
Retail pharmacy experience preferred
Required Registration/License/Certification
Kansas State Board of Pharmacy registered technician preferred
