What are the responsibilities and job description for the Home Health RN Case Manager position at SALMON Health and Retirement?
The Home Health RN provides nursing care to patients in their home and coordinates the patient plan of care with other inter-disciplinary team members.
32- 40 hour work week with flexible AM start time to support work/life balance.
We are currently hiring full-time RNs in the following areas:
- Worcester and surrounding towns
- Milford, Medway, Millis
- Westborough, Grafton
- Marlborough, Northborough, Southborough
- Provides direct care and case management of assigned patients, including initial assessment and development of plan of care.
- Evaluates patients' status and goals and revises the plan of care as needed and communicates with all appropriate team members.
- Ensures patients meet home health eligibility and medical necessity guidelines.
- Perform assigned duties, including medication administration, wound care, IV therapy, treatments, and procedures.
- Provides assigned duties, including teaching and education to patient and/or caregiver on disease process, medications, diet, wound care and other treatments as ordered by the physician.
- Completes and submits accurate, timely clinical documentation pertaining to patient interventions and care provided.
- Current license as a Registered Nurse in Massachusetts
- Minimum of two years' experience in delivering nursing care
- Minimum of one year of Home Health experience preferred
