Qualifications for Director of Social Services:

LICSW required
-
Clinical supervision and management experience preferred
-
2 years’ experience working in a SNF with short term rehab/discharge planning experience
-
Excellent communication, documentation, and assessment skills
-
Possess a genuine concern for and interest in people
Responsibilities for Director of Social Services:
-
Plan, develop, organize, implement, evaluate, and direct the social service programs
-
Build relationships between residents and staff to support resident’s individual needs
-
Find options that most meet the physical and emotional needs of each resident
-
Maintain contact with family to report changes in health, current goals, and care planning
-
Make referrals and obtain services from outside entities
-
Provide or arrange needed counseling services
-
Assist residents with financial and legal matters
-
Review facility policies and procedures as part of the facility's interdisciplinary team to assure compliance with state and federal regulations
