Hospice RN Responsibilities:
- Deliver skilled, compassionate care to terminally ill patients and their families
- Serve as a Case Manager and collaborate with the Interdisciplinary Team to establish and manage care plans
- Assess and manage patient comfort, including pain management and emotional support
- Provide education and emotional support to both patients and families during this difficult time
- Coordinate hospice services across all settings to ensure quality patient care
- Communicate with physicians and other healthcare providers as needed
- Registered Nurse (RN) with a valid California state license
- At least one year of hospice or home health experience preferred
- Background in medical/surgical, ICU/CCU, or ER nursing is a plus
- Valid driver’s license, reliable transportation, and auto insurance
- A heart for hospice care and strong interpersonal skills
- Understanding of the hospice philosophy and the needs of the terminally ill
- Self-motivated and capable of working independently with little supervision
- Current Physical and TB test results required
