What are the responsibilities and job description for the Case Manager position at SalusCare, Inc?
Full-time or part-time positions available!
This is a hybrid work position, with time primarily split between home and community work. Office requirements are minimal and limited, but can be adapted based on your work preference. This position has a flexible schedule Monday – Friday, with no on-call hours or weekend hours required. Requires an ability to work independently and maintain work requirements within required time frames.
Case managers work with a wide variety of individuals who may be diagnosed with a mental health, substance use, and/or co-occurring disorder using a strengths based approach. Case Managers may also work with individuals who may have/had criminal justice involvement, high utilization of services, pregnant woman or women with dependent children. Efforts are made to effectively link them to needed community resources, coordinate such resources as necessary, and monitor the implementation of such. Examples of such resources may include, but are not limited to, psychiatric services, substance abuse services, rehabilitation services, Supplemental Security Income (SSI), Medicaid, Patient Assistance Programs, employment, assistance with legal Issues, transportation and social supports.
We offer mileage reimbursement for all community travel done on behalf of a patient. Company laptop and iPhone are provided.

Bachelor’s degree from an accredited university or college with a major in counseling, social work, psychology, criminal justice, nursing, rehabilitation, special education, health education, or a related human services field (a related human services field is one in which major course work includes the study of human behavior and development)
And one year of full time or equivalent experience working with adults experiencing serious mental illness.
Must possess a valid Florida driver’s license and the appropriate level of automobile insurance coverage. Must have a reliable vehicle.
Computer skills in Windows operating systems required.
Training Requirements:
Complete AHCA-approved mental health targeted case management training within three months of initially providing Medicaid services.
Within 1 year must obtain Certified Behavioral Health Case Manager Certification from Florida Certification Board.
