Job Summary
We are seeking a compassionate and skilled social worker to join our Hospice team at Samaritan Health Services.

Our team serves patients in various settings, including patient residences, skilled nursing facilities, assisted living facilities, and adult foster homes.
- Provide Skilled Nursing Visits: Collaborate with healthcare professionals to assess and address patients' physical, emotional, and spiritual needs.
- Pain Management: Work with patients and families to develop effective pain management plans and address symptoms related to the terminal illness.
- Medical Social Services: Conduct assessments, develop care plans, and connect patients with community resources to ensure seamless transitions between care settings.
- Counseling and Spiritual Support: Offer individual and family counseling, as well as spiritual support, to help patients and families cope with the end-of-life process.
- Bereavement Support: Provide grief counseling and support to patients' families after the loss of a loved one.
