Job Description
Job Title : Clinical Social Worker and Chaplain
Job Specialty : Behavioral Health, Social Worker - Clinical, Chaplain
Job Duration : 13 months
Shift : Day shift, 8 : 00 AM - 4 : 30 PM, occasional weekends
Guaranteed Hours : 40 hours
Experience : Minimum of 2 years in grief or loss counseling
License : Valid driver's license and auto liability / property damage insurance as required by law
Certifications : Healthcare Provider Level BLS
Must-Have : Bachelor's degree in social services, counseling, or psychology-related area; master's degree preferred. Recent experience in hospice is preferred, with additional experience in a medical setting being advantageous.
Job Description :
- Provide emotional and bereavement assessments, counseling, resource identification, and facilitation for families during a 13-month bereavement period.
- Serve as a lead / resource for department staff and act as a subject matter expert.
- Monitor and train staff ensuring regulatory requirements are met.
- Support Hospice teams in offering services in home settings including residences, skilled nursing facilities, assisted living facilities, and adult foster homes.
- Deliver hospice services such as skilled nursing visits, pain and symptom management, medical social services, counseling, spiritual support, hospice aides, trained hospice volunteers, and bereavement support.
- Manage medication and equipment as appropriate.
- Use personal automobile for work-related travel.
