Hospice Social Worker Opportunity
We are seeking a compassionate and skilled social worker to join our Hospice team at Samaritan Health Services.
As a Hospice Social Worker, you will provide emotional, psychological, and spiritual support to patients and their families in the final stages of illness. Our team serves patients in Linn, Benton, Marion, Polk, Lincoln, and Tillamook Counties, offering services in various settings including patient residences, skilled nursing facilities, assisted living facilities, and adult foster homes.
Key Responsibilities:
- Provide Intermittent Skilled Nursing Visits: Collaborate with healthcare professionals to assess and address patients' physical, emotional, and spiritual needs.
- Pain and Symptom Management: Work with patients and families to develop effective pain management plans and address symptoms related to the terminal illness.
- Medical Social Services: Conduct assessments, develop care plans, and connect patients with community resources to ensure seamless transitions between care settings.
- Counseling and Spiritual Support: Offer individual and family counseling, as well as spiritual support, to help patients and families cope with the end-of-life process.
- Bereavement Support: Provide grief counseling and support to patients' families after the loss of a loved one.
Requirements:
- Oregon LMSW (Masters-level Social Worker) or CSWA (Clinical Social Work Associate) licensure is required for this position.
- A Healthcare Provider Level BLS certification is also necessary.
- While an Oregon LCSW (Licensed Clinical Social Worker) certification is preferred, it is not mandatory.
- Previous experience as a social worker in a hospital or medical setting is highly desirable but not essential.
