What are the responsibilities and job description for the Community Resource Navigator position at Samaritan Community Center?
The Community Resource Navigator assists clients that are experiencing challenges in food insecurity, housing instability, homelessness, lack of employment, financial crisis, etc., treating every client with dignity and compassion.
Community Resource Navigator assess our neighbors’ immediate and long-term needs, provide in-depth assistance by providing SCC resources and support our clients through the enrollment process for government services, and provide needed referrals to NWA area organizations. The Community Resource Navigator position will require excellent listening and communication skills, assessment and discernment skills, knowledge of available community resources, (including culturally appropriate educational information), the ability to manage confidential information, and exceptional organization and multi-tasking abilities.
Responsibilities:
· Performs an assessment with the neighbors to collect information and assess their immediate needs to appropriately advocate.
·Provides in-depth assistance by providing SCC resources and supporting our neighbors through enrollment with government agencies to receive benefits such as SNAP, Medicaid, and HUD Housing.
·Follows up with neighbors to ensure their needs have improved or have been resolved.
·Provides referrals for individuals to appropriate outside organizations in the NWA area and follows up to ensure the individuals' situation has improved by this referral.
·Stay current on the availability of grant funding within the organization and the community, utilizing grant funding to assist neighbors’ needs.
·Stay current on resources in the NWA community to update the resource list for all SCC staff.
·Utilizes data tracking procedures and platforms to accurately track grant spending within the Care program.
·Utilizes data tracking procedures and platforms to track neighbors’ data and success stories for program statistics, growth/development, and grant proposals.
· Develops a trusting and positive working relationship with neighbors, their families, and outside providers/organizations in NWA building both individual and community.
·Provides long-term case management for a select number of neighbors who require long-term support.
·Promotes unity between the Care program and SCC programs through collaboration, teamwork, and good communication.
· Represents SCC through outreach by promoting and developing collaboration with SCC within the community and other organizations.
