What are the responsibilities and job description for the Acute Rehab Case Manager position at Samaritan Health?
Pay Range $36.54 - $60.29
Case managers are healthcare professionals who work with patients, their families, and the medical team to coordinate and manage care. Case managers evaluate the medical, social, emotional, and financial needs of patients. They conduct assessments to determine the appropriate level of care required, both during their acute rehabilitation needs. The position requires a high degree of organizational skills along with the ability to set assignment priorities and work independently with a minimum amount of supervision. The person must be knowledgeable of community resources and able to tactfully interact with the public and staff. A high regard for confidentiality is required.
· Graduate of an accredited school of nursing.
· Minimum of two years’ experience in medical/surgical nursing or equivalent experience.
· One year of critical care is preferred, not recommended.
· Ability to correlate statistical information and outline necessary steps involved in problem solving. Aptitude for repetitive activities.
· Ability to work with doctors, nurses, social workers, and other healthcare providers to create and implement a care plan that addresses the patient’s specific needs.
· Ability to help patients and families access resources, including financial assistance, transportation, community services, and support groups.
· Ability to communicate effectively with medical professionals of all disciplines.
· Clear verbal and written communication skills, excellent phone etiquette.
· Ability to work efficiently and effectively under deadline pressures and to simultaneously handle multiple projects.
· Must be proficient in the use of Microsoft Office suite, other personal computer applications, databases and relevant software applications.
· Ability to identify potential and actual patient occurrences that may be reportable incidents or those requiring further internal investigation.
· Ability to help and prepare a patient for discharge. This involves arranging for home care, rehabilitation services, follow-up appointments, and any necessary medical equipment.
· Current NYS licensure as an RN.
· Bachelor’s degree in a related field required.
· Recommended: National Case Management certification within 3 years.
