What are the responsibilities and job description for the Intake LPN Samaritan Home Health position at Samaritan Health?
Pay Range: $24.16 - $38.65
The licensed practical nurse provides all intake services and scheduling services in the home health agency office and assist registered nurse in providing direct care. Provides follow up phone calls, and documentation in EMR. Provides patient/family teaching. Assists in daily office functioning. The LPN is also responsible for those additional skills designated by home health policy and the New York State Education Department. Performs phlebotomy, wound care and body fluid testing. Uses an understanding of the principles of growth/development to collect data on each patient's age specific needs and provide age specific treatment and care.
Job Requirements
Minimum Education Requirement :
Graduate from state approved practical nursing program. Knowledge of new trends and techniques in Nursing.
Minimum Experience Requirement :
Previous experience helpful. Possesses a thorough knowledge of nursing principles as well as SHS policies and procedures; demonstrates ability to provide quality patient care; communicates and works effectively with people; possesses ability to collect data on patient’s needs, i.e., clinical, teaching, mental and spiritual. Ability to read and communicate effectively in English. Organizational ability. Effective interpersonal, oral and written communication skills. Must possess and demonstrate knowledge of the growth and development and means to provide appropriate care to the varied age groups, pediatric to geriatric, to whom health care is provided.
Minimum License Requirement :
Current licensure or permit to practice issued by the New York State Education Department. BLS required.
