Essential Functions
Provide direct oversight of hospice care provided by facility-based hospice teams and staff to foster quality patient and family outcomes
- Collaborate with managers/directors/VPs to continuously monitor quality of care and clinical outcomes; collaboratively develop strategies to improve patient care delivery
- Develops and monitors FTE and expense budgets in collaboration with VP of Hospice Services
- Develop and support strategic programs and initiatives to ensure hospice growth in these facilities
- Maintain communication with referral sources and in collaboration with Business Development and other Samaritan business lines, continually monitor
- Collaborate with VP of Hospice Services and VP of Quality and Compliance managers/directors to assure regulatory compliance measures are met in accordance with the Medicare Conditions of Participations for Hospice and the Joint Commission Standards of Care for Hospice Accreditation
- Ensure that internal and external customer needs are addressed and met.
- Investigate and resolve facility/patient/family/staff complaints
- Participate in the hiring, orienting and the on-going assessment process of on-boarding new staff into both clinical and managerial roles at Samaritan
- Communicate effectively in all settings and with all levels of personnel
- Serve on clinical and administrative committees as appropriate
- Maintain open line of communication with VP of Hospice Services who is Hospice Director’s Administrator
- Other responsibilities as assigned
Qualifications:
Education or Degree Required: RN required. BSN, MSN preferred.
- License/Certification Required: NJ RN License required, valid NJ driver’s license and auto insurance
- Related Experience and Training Requirements: 7- 10 years nursing experience in a variety of settings. A minimum of 3 years progressive leadership experience in Long Term Care, required.(Consideration may be given to other progressive hospice leadership experiences) Professional work experience in hospice, preferred.
- Skills and Abilities: management skills, understanding of/and commitment to hospice philosophy, ability to manage and encourage change, excellent communication skills, and ability to analyze data and metrics.

- Required Time frame to learn the job once hired: Approximately 6 months
