Clinical Nurse Specialist - Home Health

Job Overview
We are seeking a dedicated and compassionate Registered Nurse (RN) to join our Home Health team. In this role, you will provide high-quality nursing care to patients in their homes, ensuring they receive the support and medical attention they need for optimal recovery and well-being. The ideal candidate will possess strong clinical skills, excellent communication abilities, and a commitment to patient-centered care.

Responsibilities

• Conduct comprehensive assessments of patients’ health status and needs in their home environment.
• Develop and implement individualized care plans in collaboration with patients, families, and healthcare teams.
• Administer medications and treatments as prescribed, including ostomy care and specimen collection processing.
• Monitor patient progress and adjust care plans as necessary to ensure effective outcomes.
• Provide education to patients and families regarding health management, including family planning and behavioral health strategies.
• Document all patient interactions accurately in medical records, adhering to medical terminology standards.
• Collaborate with interdisciplinary teams to coordinate urgent care needs and emergency medicine interventions when required.
• Maintain compliance with all healthcare regulations and organizational policies while ensuring patient confidentiality.

Experience

• Current Registered Nurse (RN) license in the state of practice.
• Previous experience in home health or community nursing required.
• Familiarity with urgent care practices, emergency medicine protocols, and inpatient care is advantageous.
• Skills in suture techniques and behavioral health management are a plus.

Job Type: Full-time

Pay: From $60.00 per hour

Expected hours: 32 – 40 per week
