Health Home Medical Social Worker

Job Overview

We are seeking a dedicated Medical Social Worker to join our team. The ideal candidate will provide essential social work services to patients in a medical setting, offering support and assistance during challenging times.

Duties

- Collaborate with medical staff to assess patients' psychosocial needs and provides community resources.

- Provide guidance and emotional support to patients and their families

- Conduct psychosocial assessments

- Maintain accurate medical documentation and data collection

- Intervene in crisis situations and provide crisis management support

- Ensure compliance with HIPAA regulations and ethical standards

- Utilize psychotherapy techniques when working with patients

Competencies

- Must be proficient in the use of Microsoft Office Suite.

- Must be capable of accurate documentation within the Electronic Health Record.

- Must have excellent observation, good clinical judgement, oral and written communication skills.

- Must possess the ability to deal tactfully with personnel, patients, family members, visitors, government agencies/personnel and the general public.

- Has good organizational skills.

License, Education, and Experience Requirements

- Has completed a masters or doctoral degree in social work or related field.

- One year of clinical experience; home health experience preferred.

- Demonstrates knowledge of resources available in the community.

Additional Requirements

- Clear criminal background check.

- TB test.

- Health screen clearance.

- Completion of the credentialing process.

- Must have a valid in-state driver’s license, provide current registration and insurance, and have a driving record with no more than two points within the last three years.

Pay: $38.00 - $45.00 per hour
