Job Description

Job Title : Registered Nurse, Care Manager

Job Specialty : Nursing

Job Duration : 13 weeks

Shift : Days (0730-1700), rotating weekends

Guaranteed Hours : 40 hours per week

Experience :

Minimum of 3 years of RN experience required
3 years of hospital nursing experience preferred
Acute care utilization review, discharge planning, or case management experience preferred

License :

Current unencumbered RN Licensure required

Certifications :

Healthcare Provider Level BLS required

Must-Have :

Experience or training in insurance benefits or covered services, including Medicare and Medicaid

Utilization Review or inpatient Case Management certification preferred

Job Description :

Collaborate with a multidisciplinary team to provide direct social services and interventions for clients

Screen and assess clients, make referrals, and impart essential information as needed

Intervene in crisis situations, utilizing expertise within the area of practice

Conduct acute care utilization review and participate in discharge planning

Work closely with doctors, nurses, and other medical professionals to ensure comprehensive patient care

Maintain patient records using EPIC EMR system

Provide holiday coverage as required

Engage in continuous learning and up-to-date practice within the field of nursing care management
