We are looking for a board-certified Family or Internal Medicine Physician to join our esteemed team in Derby, CT , and surrounding areas. In this role, you will provide high-quality, patient-centered care in an office-based setting, managing both acute and chronic conditions while working collaboratively with a dynamic team of healthcare professionals.
Key Responsibilities :
- Patient Care : Examine patients, obtain medical histories, and diagnose and treat both acute and chronic illnesses.
- Diagnostic Expertise : Order, perform, and interpret diagnostic tests to assist in the treatment and management of various health conditions.
- Care Plans : Develop comprehensive treatment plans, involving patients and their families in the decision-making process.
- Specialty Referrals : Initiate appropriate referrals to specialists when needed to ensure complete care for your patients.
- Collaborative Approach : Work closely with the care team to improve service delivery, address care gaps, and enhance quality measures.
- ACO and PCMH Initiatives : Actively participate in Patient-Centered Medical Home (PCMH) and Accountable Care Organization (ACO) initiatives.
- On-Call Coverage : Provide on-call coverage within the primary care group as needed.
- Compliance and Documentation : Ensure accurate patient documentation, and work with management to ensure compliance with billing and coding standards.
