Job Summary
We are seeking a dedicated and compassionate Physical Therapist to join our Home Health Hospice team. The ideal candidate will provide exceptional care for patients in their homes within our service areas in Madera and Fresno. This role involves assessing patient needs, developing individualized treatment plans, and guiding patients through rehabilitation exercises to improve their physical function and quality of life.
Duties :
- Assessing and evaluating therapeutic / rehabilitative / functional status, and participation in the development of the total plan of care. Evaluates home environment.
- Assesses for muscle strength, mobility, gait, ROM and transfer capabilities. Less a goal of rehab, more a focus on maintaining comfort.
- Develop and implement personalized treatment plans based on individual patient assessments.
- Monitor patient progress and adjust treatment plans as necessary to ensure optimal outcomes.
- Educate patients and their families on exercises, therapeutic equipment / appliances, and home care strategies to promote recovery.
- Collaborate with other healthcare professionals to coordinate comprehensive patient care.
- Document patient progress accurately using medical terminology and maintain up-to-date records.
- Participates in patient discharge planning. Prepares and submits a clinical progress summary based on the attainment of goals directed by organization policy.
- Provides supervision of physical therapy assistants and hospice aides, as appropriate.
