What are the responsibilities and job description for the Registered Nurse Case Manager position at Samuel Contract Staffing?
I’m working with a hospice provider in Salem, OR and we’re looking for Hospice RN Case Managers to join us full-time. We want someone who is passionate, empathetic, and who is willing to partner with patients and their families to get the most out of their remaining time together.
Your Role Will Involve :
- Using your teaching skills to empower family caregivers and help them identify their options for care to optimize quality of life.
- Coordinating the care of hospice patients to meet the holistic needs of the patient and their family / caregivers.
- Providing caring, professional nursing services to hospice patients.
- Performing initial and ongoing assessments of patients, developing and revising the plan of care as needed.
- Providing timely documentation and communication of assessments, planning, and interventions.
