Overview
The Director of Patient Care Services (DPCS) plays a crucial role in overseeing the operations of a home health care and hospice agency, ensuring that high-quality care is delivered to patients in their homes. This oversight includes patient and personnel assignments, coordination of care, coordination of referrals, assessment of patient needs, and ensuring the plan of care is developed, implemented and updated. This position requires a strong understanding of healthcare regulations, patient rights, and effective management practices. The DCPS is the primary Clinical Manager for the Home Health/Hospice Programs and is essential for managing the complex, interdisciplinary care of home health/hospice patients. The ideal candidate will possess a blend of clinical knowledge and administrative skills to lead a team of health care professionals while fostering an environment that prioritizes patient well-being.
Duties
- The DPCS is responsible for staff engagement, development and performance, including policy, regulatory and compliance knowledge and adherence.
- Manage patient care services, including hospice care and critical care, while advocating for resident rights.
- Maintain accurate medical records and ensure proper documentation of patient care activities.
- Develop and manage budgets for various programs within the agency, ensuring efficient use of resources.
- Supervise staff, including shift management and scheduling to ensure adequate coverage for patient needs.
- Provide training and support to staff on medical terminology and best practices in assisted living environments.
- Collaborate with healthcare providers to coordinate patient care plans and improve service delivery.
Qualifications
- Three to five years’ experience as a Home Health Supervisor or Hospice Manager.
- Possession of a current RN license to practice in the state of California.
- Current CPR certification.
- Familiarity with budgeting processes and financial management within a healthcare setting.
- Excellent leadership skills with the ability to manage teams effectively.
- Understanding of resident rights and commitment to providing compassionate patient-centered care.

Pay: $149,840.00 - $170,370.00 per year
