What are the responsibilities and job description for the Clinical Care Coordinator position at San Carlos Healthcare Corporation?
The Clinical Care Coordinator assists providers and other clinical staff and works independently to assess physical issues, vital signs, medication adherence, and plan issues, as well as medical problems, including psychological issues of the patient, and functions in a multi-disciplinary work environment by performing the following duties.
ESSENTIAL FUNCTIONS
- Completes nursing assessment of patient’s condition consistent with age and condition including physical, psychological, social, developmental and educational needs
- Determines initial level of compliance with treatment plan in accordance with hospital policy and unit guidelines
- Performs ongoing patient/family assessment and identifies variances from diagnosis
- Maintains and updates patient’s medical records through concise, accurate and timely documentation
- Works collaboratively with patient care team and family members to establish and implement plan of care to ensure coordination of activities and ongoing communication of patient health status
- Participates in patient care rounds and patient care conferences as appropriate
- Continually assesses and evaluates patient’s health status and response to care, teaching and discharge plan
- Shares observations and assessment outcomes with health care team and family to ensure communication and understanding of patient health status, progress and ongoing needs
- Identifies, removes, minimizes and/or manages health issues of students to ensure students remain in school
- As primary nursing care provider, supervises patient care activities of assigned team members, delegates activities appropriate to knowledge, skill and competency; provides timely feedback and review of care delivery to ensure maintenance of patient safety, infection control procedures and therapeutic environment
- Maintains professional and technical competency through regular participation in a variety of educational programs and in-services, reviews relevant professional publications and materials
- Develops and maintains good working and professional relationships with other health care team members and providers to facilitate open and cooperative communication
- Provides orientation to the unit and hospital systems and processes to patients and family members to ensure familiarity with unit policies and requirements
- Contributes to the team/unit’s efforts by completing required activities within established parameters to ensure compliance with all applicable medication, storage and documentation standards, regulations and requirements
- Reviews and reinforces with patient/family ongoing education of basic technical procedures, activities and safety necessary for daily living
- Participates in departmental orientation, on the job training and quality assurance programs/initiatives
- Participates in a variety of department and hospital educational programs to maintain current skills and competency levels; identifies and discusses performance or training needs with Supervisor
- Performs other duties as assigned
